Terms and conditions for ONLINE booking
of appointment at Prayas Clinic
- Consultation with Dr. Dipankar Sen is on the basis of appointment - which is available online at website www.prayas-foundation.com. Appointment must be booked also for review of reports.
- Online appointment is available one week in advance and no later than 9 PM of the day before consultation.
- Any unoccupied appointment slot can be booked by telephone or visiting the clinic on the day by first come first serve basis from 9 AM onwards.
- You should arrive at least 15 minutes before your time of appointment and report to reception with the booking confirmation receipt or SMS confirmation.
- The fees for consultation with Dr. Dipankar Sen and Dr. Dipanwita Sen is Rs 800 .
- The fees for review of reports for both the doctors is Rs 400 for one week after the primary visit, after this full consultation fees will be charged
- The fees for ex SAIL employees and ex Serviceman/woman is Rs 200 for all visits.
- Patients with emergency or children or very old people may be given priority on the discretion of the doctor.
- You must bring all treatment records and investigation reports on all occasions as doctor might need to see it repeatedly.
